Transmitting patient information via E-Communications has a number of risks that you should carefully consider before using E-Communications to communicate with us or agreeing to allow us to use E-Communications to communicate with you. • E-Communications should not be used for highly sensitive communications.
• You are responsible for informing Provider, in writing, if you want to cease or limit E-Communications with Provider. You may do so at any time without reason or explanation.
• You are responsible for protecting your E-Communications account password or other means of access.
• Limit or avoid the use of public computers and public networks.
• Promptly inform Provider of changes in your E-Communications address.
• Take precautions to preserve the confidentiality of your E-Communications. Once Provider sends an E-Communication from its network, it has no control over its confidentiality or security.
